I have done all the steps

I did this and when I went to refresh the Leads page, it wouldn't show up "Miles Dyson"

The list of leads you are looking at in your 2nd screencap above is "Recently Viewed". That means it is showing you leads that you viewed already (recently). Since you haven't already examined the Miles Dyson lead record, it wouldn't show up in that list view.
The beginning of Step 140 says to change the list view to "Today's Leads". Then it would list Miles Dyson because that lead got created today. But if you do this tomorrow, then you won't see it because he was added today. So tomorrow, you will need to add him again and then view "Today's Leads" and he will show up. (You will then have 2 Miles Dyson lead records but only the one from the same day will show up in Today's Leads. That's ok.)
I finished step 137. The thank you message played. But I didn't receive the two emails. In addition, Miles Dyson doesn't show after I refresh many time. Please help.
Ok, do these 3 steps so I can access your account and also email me at tim.hill@themaxlabsproject.com and include your web2lead.html file as an attachment.
Log in and click on the character icon (upper right) and click My Settings
On the left, under My Personal Information, click Grant Account Login Access below that
On the row for The Max Labs Project Support, use the --No Access-- pull-down menu to select 1 month and click Save
I am having the same exact problem where I haven't recieved the thank you emails and Miles is not showing up in my list even when I have it on all open leads and even by searching his name the name does not appear
Ok so do those same 3 steps above and also email me at tim.hill@themaxlabsproject.com and attach the HTML file you’re using (myweb2lead.html) and I’ll take a look.
