Ok, I logged into your account and found the issue. I was wrong before.
There was nothing wrong with your auto-response rule. The thank-you email was being sent when you hit Submit and that was showing up on the lead timeline. But your workflow rule was wrong so it wasn't creating the follow-up task for you to to in the future so that's what was missing after 144.
In Step 46 where it says:
"enter “150000” (4 zeros—make sure!)"
you only put in 2 zeros. So it was set to trigger only if the Investment Potential was less that $1,500. When you put in Miles Dyson at $78,000, it didn't trigger because $78,000 is more than $1,500.
Go back and edit your workflow rule you built in Steps 33-47 and fix that value. Then, delete the lead Miles Dyson and re-add it again using your Web2Lead.html form and it will work this time.
