Step 126
Upon reaching step 124 of the document I was met with the "Contact" box being missing from the form. I am not sure where or how I went wrong along the way and have no current way of knowing what I need to fix for this to show up.

Good work finding a solution to your initial issue! For this next one, with the Email data being missing, I'm guessing there's something wrong with the formula you gave for that field but hard to tell without seeing your account. If you do these three steps:
Log in and click on the character icon (upper right) and click My Settings
On the left, under My Personal Information, click Grant Account Login Access below that
On the row for The Max Labs Project Support, use the --No Access-- pull-down menu to select 1 month and click Save
then I can check your account and let you know. Please let me know when you've completed them and then I'll check it out, ok?
