Lab 1a

Hello all! Need some help with my lab 1a, as it shows me "my visible fields is set to the wrong ordering", where it is adding underscores to my field name. I have compared and gone over the assignment with my professor, and he completed his the exact same, where only I am getting the error. I have formatted everythijg correctly, so I am not sure why the interface is pulling my field labels with underscores. Any help would be appreciated!
