When I click the myweb2lead.html link, it sends me to a blank screen.
I am using Windows, please let me know how I should proceed.

Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Ok, I got your file. Thanks. I see what happened. Your file only has the one line that you copied in Step 99:
<form action="https://webto.salesforce.com/servlet/servlet.WebToLead?encoding=UTF-8" method="POST">
But in Step 104 you were supposed to copy that whole chunk of HTML code that it shows in the screencap right under 104 but somehow you missed that so you pasted in just that one line.
Do you still have that whole chunk of HTML displayed at this point so you could copy it now and paste it into your file? If so, just delete your current myweb2lead.html file and do Step 104 and go from there. If not, then redo Steps 89-98 to get that chunk back and then go on from Step 104.
