I didn't receive the email. I tried backtracking to classic email template but I can't seem to figure out what the issue is.

Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, uninstall Max's Pitch Package, etc. That can make it worse. There's a 95% chance the same snag/fix is listed & answered in the Help FAQ. But if you can't find it there, post your problem here.
Make sure you post in the right category below & be sure to include a screencap showing the issue!
It's probably a problem with the auto response rule. Check that out and if you still can't get it to work and do these three steps so I can access your account and take a look. Let me know when you've done them.
Log in and click on the character icon (upper right) and click My Settings
Click My Personal Information (upper left) and then Grant Account Login Access below that
On the row for The Max Labs Project Support, use the --No Access-- pull-down menu to select 1 month and click Save

Ok but I have not been granted access to the account with the name "Nhien Ha" on it. Please redo those 3 steps and make sure you click Save at the end.

Ok, so I just added myself as a lead and it worked fine. I got the email and the email is listed as being sent to me when you look at my lead record. Are you sure you added Myles Dyson using the HTML form, opened in your browser? If not, it won't work. You have to add him the way the instructions say, using the HTML form you create in those steps. You can delete Myles and try adding again using the HTML form.

Is this the HTML form you're referring to?
Yes, but when you add the lead, you need to add a valid email address in that slot so Salesforce can successfully send the email to that person (can be your own).
When I added myself as a lead that way, using that form, putting in my own email address, it all worked fine!

if my last name only consists 2 letters, do i just go with that?
No you don't have to do that part again. Just delete Myles Dyson and then use the HTML form you have to add Myles Dyson again but this time, enter your own email address into the form so you can get the email when you click Submit.

Well, now it's not working for me either. But it did work for me this morning. Did you change any settings at all today? Or just add Miles Dyson?
I just saw that the rule's Active checkbox was unchecked. Did you uncheck it today? I just re-checked it. It should work fine now. I will test it now.
