I was able to fill in the form. The video never played and instead a 404 not found page popped up stating that the "server cannot find the requested page." Therefore, I received an email stating "Salesforce could not create this lead because of the reason listed below.

Well, the reason you got the 404 error was that the retURL you put in is missing one of the "w"s. It is supposed to be three "w"s right? Like "www.signingsavvy.com....." etc.
Fix that and then post again if you're still having problems, ok?

But I still received an error email from Salesforce stating "Salesforce could not create this lead because of the reason listed below. Actually, it's fine. Even though I received that email, I checked my leads page and Miles popped up!
@Lily Hang Oh, good. Algd to hear it. I was just looking at that and I think this error message comes from trying to add a lead that is a duplicate of one that is already there. So probably when you got the "www" error for the thank you, the lead did get created anyway. Then when you tried again, it was giving the lead capture error due to Miles already being there when you tried to add him again.
Anyway, all good now. I'm glad.
