Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.
Before you post make sure you check the Help FAQ for an instant solution first. Then, if you can't find it there, you can post your problem here and we'll get to it ASAP (within 24 hours), but be sure you:
post in the right category

Nancy, this email needs to be the same as the one on your user account. Go to Setup > Users > Users and click on your name and you'll see all your info. Check the email address listed there for you. Is it correct? If not, click Edit and change it and then click Save. But then you will get a verification email at that address so then you need to open that email and click the link to verify and only then that new email address will be saved.
Then you should be able to redo Steps 71-88 and it should work.
1. First skip the (EMAIL ADDRESS:) ...leave blank
2. Then skip to below:
Under Step 4: Select the template to use, click the magnifying glass next to Email Template to bring up the Lookup popup window and click auto-response template for incoming webleads for me to populate it into the textbox
3. Go back up to EMAIL ADDRESS (registered w/salesforce) and ENTER MANUALLY (write out your email address)
4. SAVE!! <3
