Sorry you're stuck but there's almost always a pretty easy fix! Do NOT try to start all over again, eg. uninstall Max's Pitch Package, etc. That can make things worse.

Before you post make sure you check the Help FAQ for an instant solution first. Then, if you can't find it there, you can post your problem here and we'll get to it ASAP (within 24 hours), but be sure you:

Step 142

in Lab 2b

I am on Step 132 and when I press the gear icon, there is no way to click "select fields to display" like it is asking me to do.

After 141, you should have been viewing the new view you just created called "Contact Info for Mountain View Reception" but you're showing "Recently Viewed" somehow, so click on that (Recently Viewed) and select "Contact Info for Mountain View Reception" then you will be able to do 142.

But you never got back to me on how you solved the issue with the Contact Name slot on that form. Did clearing the cache work?
